Ontario’s 14 Home and Community Care Support Services organizations coordinate in-home and community-based care for thousands of patients across the province every day.

Lisa Blundell RN BN, Care Coordinator with Home and Community Care Support Services Central West, will be joining us for Coffee Hour on Sunday May 19th. Lisa is a strong advocate for making plans before a crisis.

Whether it is for ourselves or for a loved one, Lisa will spend about an hour informing us about:

  • Remaining safely at home with the support of health and other care professionals
  • Returning home from hospital and recover at home
  • Finding a family doctor or nurse practitioner
  • Finding community services that support healthy, independent living
  • Transitioning to long-term care or supportive housing
  • Anything else that might be on our minds regarding the supportive process as we age
